Post-partum Home Visits

When clients are discharged from the hospital Public Health Services receives a birth referral (or notice) from the hospital. Public Health Nurses contact the family and complete a brief telephone assessment and arrange for a home visit.

Women and their babies after giving birth

When and where is it offered?

The Public Health Nurse arranges a visit to your home 1-2 days after hospital discharge.

Does this program require a referral?

The hospital sends a notice to Public Health Services. Anyone can self refer by calling a Pubic Health office nearest them if they haven't been contacted post birth
