Community Cardiovascular Hearts in Motion (CCHIM)

This is a twelve week program, and each week you are required to attend two one-hour exercise classes. Also, you must commit to a one-hour education session per week.

The Community Cardiovascular Hearts in Motion Program offers:

  • Individual Assessment by a Nurse, Physiotherapist and Dietitian
  • Supervised Group Exercise Program
  • Nutrition Assessment and Personal Nutrition Plan
  • Lifestyle Change
  • Realistic Goal Setting Approach
  • Home Exercise Plan
  • Educational Sessions Designed to Improve Your Lifestyle

The educational sessions include topics such as:

  • Risk Factors for Heart Disease
  • Heart Healthy Eating
  • Exercise
  • Medications
  • Managing Stress
  • Coping Skills

Who is the program for?

This program is for people with heart disease, cerebral vascular disease (i.e., stroke), and peripheral vascular disease (i.e., poor circulation to the legs). The program is also for people who have risk factors for heart disease such as diabetes, high blood pressure and high cholesterol.

To find out if you can take part in the Community Cardiovascular Hearts in Motion Program consult your family doctor and ask for a referral.

The CCHIM Program is mobile and is offered in one community at a time throughout the Guysborough Antigonish Strait Health Authority.

The CCHIM program has been offered in the following communities:

  • Port Hawkesbury
  • St. Peter's

Does the program require a referral?

Yes. Contact your family doctor and request that a referral be sent to the Community Cardiovascular Hearts in Motion Program.
